MAIL TO
Dashed lines are for you to fill in, in the department's own words.
Put the full name and KDOC number first, then facility name, address, city, state and ZIP. Add your own name and return address or the mail won't be delivered. Send only letters or photographs, no stamps. Two ZIPs are listed (67550-9304, 67550-0280), so confirm which is right.
Legal mail from lawyers and courts has its own address or rules: see the department's mail page.
